On your initial appointment, we will perform specific testing on your tooth to determine the best treatment of choice for your individual situation. The findings will be explained to you in detail and treatment can be done on the same day.
Prior to your first appointment, you can download and complete the online Health History and Consent Forms, otherwise plane to arrive for your appointment at least 15 minutes early to complete the necessary paperwork.
If premedication is necessary for an existing medical condition, please inform us of this at the time you schedule your appointment. If you are not an existing patient of record, you may have to contact your general dentist to prescribe these medications for you.
Dental insurance information will be obtained prior to your first visit. We will make every attempt to inform you of your estimated co-payments, if any, prior to your appointment.
IMPORTANT: Parents or guardians must accompany patients under the age of 18 years of age at the consultation visit.
EX-RAYS: If your dentist has taken x-rays, please bring us a copy or have them e-mailed to us at [email protected] prior to your appointment. We will take necessary images if x-rays are not available. Additional films may be taken at our facility if necessary.
